Twenty years ago, on one of our first outings together, I bought Sofia nasi lemak daun pisang from a famous stall in Ampang. She took one bite of the sambal — the level of heat a Malay kindergartener would eat without blinking — and her face and ears went deep red. She could barely finish it. It’s a common story for many Chinese Malaysians who didn’t grow up eating chilli daily.

Over the years together, that changed completely. She grew to genuinely love chilli. Except for one thing that made no sense: certain chillies would trigger a migraine. Not all of them. Just some. This was never a blood pressure issue, then or now — it was always specifically a migraine, and it showed up long before blood pressure was ever a topic in this household.

The answer only showed up after I started growing my own cili padi — several varieties, fully organic, aquaponics water from my patin and catfish tanks, pest control from wood vinegar, neem oil, garlic water, coriander spray, and Metarhizium anisopliae — a naturally occurring insect-targeting fungus — instead of any synthetic spray. She ate every single variety. Not one migraine. That single finding pointed somewhere neither of us expected: maybe it was never the chilli at all.

The Same Lock Also Works in Reverse

That’s a real, documented pathway from chilli to headache — capsaicin genuinely can trigger CGRP release through TRPV1. But the same receptor has a second, opposite behaviour: repeated, chronic exposure to capsaicin eventually desensitises those same nerve terminals, which is why topical capsaicin is an actual medical treatment for migraine and cluster headache.

Twenty years of steadily building up chilli tolerance is exactly the kind of chronic exposure that would produce that desensitisation. Which made the sudden, specific migraine trigger even more of a puzzle — until the variable turned out to be something other than capsaicin.

The Small Chilli, Not the Rice-Paddy Chilli

Capsicum frutescens did not evolve in a Malaysian kampung garden. It’s native to Central and South America, and arrived in Southeast Asia with Spanish and Portuguese traders. The name marks its size, not its habitat — it’s the small chilli, set against the much larger cili merah and cili hijau. Its other common name, cili burung — “bird chilli” — points at the same fact from a different angle: small enough, and unbothered-by-capsaicin enough, that birds do most of the work spreading its seeds.

Less Heat, Same Flavour

Most of cili padi’s capsaicin sits in the white pith and seeds, not the flesh. Split and scrape these out if you want the aroma without the full burn.

Traditional Preparation

Antiseptic and fever-remedy use is documented for the fruit taken in small amounts — but exact home preparation methods vary by household and region, so this article doesn’t prescribe a specific recipe for that use.

Storage

Don’t wash cili padi before refrigerating — surface moisture speeds up rot. Wash only right before use.

Freezing

Cili padi freezes whole with almost no loss of heat or flavour — useful for smoothing out supply during off-season price spikes.

Hands After Handling

Capsaicin is oil-soluble, not water-soluble. Plain water won’t fully clear it off your skin — a little oil or dish soap first, then water, works better. Never touch your eyes in between.

What People Believe About Chilli — and What’s Actually True

■ The Viral Claim

“Chilli triggers migraines.”

There is a real mechanism here, and it isn’t imaginary. Capsaicin activates TRPV1 on the trigeminal nerve — the nerve now understood to drive most migraine attacks — and that activation can trigger release of CGRP, the exact molecule the newest generation of migraine drugs is built to block. Capsaicin-to-migraine is a documented pathway, not folk belief.

But the same receptor has an opposite long-game behaviour: repeated, chronic exposure to capsaicin desensitises those nerve terminals over time — precisely why topical capsaicin is an actual clinical treatment for migraine and cluster headache. Twenty years of steadily increasing chilli tolerance is exactly the kind of chronic exposure that produces this desensitisation. That’s consistent with someone growing more comfortable with heat over two decades, not less.

Which leaves the sharper question: if capsaicin itself should, by this point, be desensitising rather than triggering, why did specific chillies still cause migraines — and why did that stop completely on fully organic, no-pesticide, no-chemical-fertiliser homegrown chilli, across every variety? The separate, non-capsaicin explanation has its own documented support: environmental chemical exposure, including pesticides, is independently linked to migraine and headache, and at least one pesticide has been shown in lab studies to activate TRPA1 — a different receptor also implicated in migraine — and to trigger CGRP release on its own, with no capsaicin involved at all. Two separate pathways can converge on the same nerve. This has not been lab-confirmed for any specific pesticide or chilli batch in this case — it’s the strongest available explanation, not a proven one.

Real mechanism, overstated size. Capsaicin activates a measurable thermogenic response — it raises resting energy expenditure slightly and, at consistent 8-week dosing, increases brown adipose tissue activity. A meta-analysis found capsaicinoids taken before a meal reduced how much food people ate afterward by roughly 74 kilocalories.

That’s a real, receptor-level effect — and nowhere near enough to “melt” anything on its own. The honest framing: capsaicin is a minor assist that can shave a small margin off intake and energy expenditure. It is not a substitute for what actually drives fat loss.

■ The Viral Claim

“Spicy food causes stomach ulcers.”

✗ Busted

Capsaicin does not cause ulcers — that idea has been thoroughly displaced by the discovery that H. pylori bacteria and NSAID use are the actual drivers. In people without an existing ulcer, capsaicin in food-level amounts stimulates gastric juices and appears to have a mild protective effect on the stomach lining, not a destructive one.

The nuance worth keeping: in people who already have GERD or an active ulcer, capsaicin will aggravate symptoms — one study even found repeated exposure eventually reduced heartburn severity compared to the first exposure, suggesting some adaptation over time. Existing damage is the precondition for harm, not the chilli itself.

Honest Limitations

Well documented: TRPV1 as the capsaicin/heat receptor is a Nobel Prize-winning, thoroughly replicated finding. Its presence on the trigeminal nerve and its role in CGRP release are established science. Capsaicin desensitisation as a clinical migraine and cluster-headache treatment is real and in use.

Needs honest framing: most of the strongest capsaicin-and-migraine mechanism data — TRPV1 activation, CGRP release, trigeminal sensitisation — comes from animal models and isolated nerve-fibre studies, not large controlled human trials on chilli specifically. The link between environmental chemical exposure (including pesticides) and headache is better established in occupational and epidemiological literature, but the specific TRPA1/CGRP pesticide pathway is still largely lab-based. Neither side of this story is proof for any one household’s experience — it’s the best available explanation, built from real, separate, published mechanisms.

And Sofia’s specific observation — certain chillies, not chilli in general, triggering a migraine — remains a personal pattern, not a controlled study. It has two plausible mechanisms (capsaicin-driven CGRP release via TRPV1, or a separate chemical trigger from pesticide or fertiliser residue), and no lab confirmation of which one it actually was, or whether it’s some combination of both. This is flagged as Plausible—Not-Yet-Confirmed, not settled fact.

⚠ Caution
  • Blood thinners (warfarin, aspirin, other antiplatelets): capsaicin has mild blood-thinning activity of its own. Concentrated intake alongside these medications is a real interaction, not a theoretical one.
  • ACE inhibitors for blood pressure: capsaicin can worsen the dry cough that’s already a known side effect of this drug class.
  • Diabetes medication: capsaicin can lower blood sugar. Combined with glucose-lowering drugs, monitor for hypoglycaemia.
  • Active GERD, ulcers, IBS, or inflammatory bowel disease: capsaicin will aggravate an already-irritated gut lining. This is the one group where “spicy food is fine” does not apply.
  • Pregnancy: dietary amounts in normal cooking are considered fine; concentrated supplements are not well studied and should be avoided without medical guidance.
  • If you get migraines with chilli: don’t assume it’s the capsaicin and don’t assume it isn’t. Track which specific chillies or sources trigger it — homegrown versus store-bought, organic versus conventional — the same way Sofia’s pattern only became visible over time.
  • If a specific batch of chilli seems to be affecting you differently than usual — as it did for Sofia — that is worth paying attention to. The variable may not be the chilli itself.

I still remember Sofia’s ears going bright red over a plate of nasi lemak daun pisang in Ampang, twenty years ago. That version of her would not have believed she’d one day genuinely crave chilli. But the migraines were the one piece that never added up — some chillies were fine, some weren’t, and there was no pattern we could see from the outside.

The test wasn’t a controlled experiment. That’s not proof. It’s one household, one growing season, one set of organic inputs against one set of unknown commercial ones. But it’s the kind of pattern that’s worth taking seriously precisely because it came from paying attention over years, not from a single bad meal. Whatever was triggering her migraines, the evidence so far points away from the chilli itself and toward what’s sprayed or fed onto it before it reaches the plate.
